To refer your patients to the Pilot, you must be a Primary Care Provider (FP or NP) in the Fraser Health Region.
If you meet the above eligibility criteria, please proceed to complete the RAC for LBP Pilot Registration and Onboarding Module.

Please ensure to add "1" before the fax number when sending referrals
** Please note that an email address is required to complete the online questionnaires. If your patient does not have their own email address, please ask them to provide a family member or friend's email who can assist them. Incomplete referrals will not be processed and RAC LBP Admin will not contact patient's directly for missing information.**
